התפתחות בדרך למיגור מחלת סרטן צוואר הרחם

סוגיית המחלות הנגרמות עקב נגיף הפפילומה האנושי (HPV) טרם נפתרה, כש-570 אלף נשים בכל העולם חולות בסרטן צוואר הרחם מדי שנה. סקירה מיוחדת לרגל שבוע המודעות למחלה

בשנים האחרונות נוצרה אשליה, שפתרנו כבר את בעיית סרטן צוואר הרחם ויתר המחלות הנגרמות עקב HPV – נגיף הפפילומה האנושי. אלא שהמציאות ממשיכה לטפוח על פנינו, ועדיין כ-570 אלף נשים בכל העולם חולות בסרטן צוואר הרחם מדי שנה1. כל שתי דקות נפטרת אשה, אחרי מאבק קשה וכואב, מסרטן צוואר הרחם2. זהו גידול הפוגע בנשים צעירות וכל מקרה משאיר אחריו משפחה שבורה וסובלת.

המציאות הזו טולטלה לאחרונה, כאשר ארגון הבריאות העולמי גייס 194 מדינות שנטלו על עצמן להעלים את סרטן צוואר הרחם בתוך מספר שנים3. ההכרזה על ההתגייסות הגורפת של המדינות ריגשה גם מומחים ותיקים, שכן זו הפעם הראשונה שארגון הבריאות העולמי מוביל יוזמה להעלמת מחלת סרטן. מיד גם עולה הזיכרון של ההצלחה במיגור מחלת האבעבועות השחורות שבעבר גרמה למוות בייסורים של מיליוני בני אדם4.

ברור עם זאת, שעדיין יש דרך לעשות לפני שסרטן צוואר הרחם ייעלם מחיינו. הדרך כרוכה בעמידה ברף המכונה 90-70-90. כלומר, יש להגיע לפחות לשיעור של 90% התחסנות. הכוונה היא שלפחות 90% מהבנות יקבלו את החיסון כנגד HPV לפני גיל 15. כמו כן, שלפחות 70% מהבנות יעברו משטח לסריקת צוואר הרחם באמצעות תבחין יעיל – שכיום מבוצע בעזרת בדיקת PCR לנוכחות HPV. לבסוף, שהנשים שאובחנו עם טרום סרטן או סרטן צוואר הרחם – יטופלו. לפחות 90% מהן3.

אין ספק שהיעד החדש מעורר התלהבות ותקווה לשיפור משמעותי באיכות החיים, אך הוא מיד מעלה מספר שאלות, שאנסה לענות עליהן להלן:

  1. למה הכוונה ב"אלימינציה"? האם לחיסול מלא והעלמת המחלה?
    התכנית היא להגיע תוך 10 שנים להיארעות של פחות מ-4 נשים עם סרטן צוואר הרחם ל-100,000. כך נמנע תמותה של 300,000 נשים בעולם במשך 10 שנים5.
  2. מי יממן את ההגעה ליעדים הנדרשים?
    כאמור, 194 מדינות הצטרפו ליוזמת ארגון הבריאות העולמי והתחייבו להשקיע כל מה שכרוך כדי לעמוד ביעד המשותף. למעשה, בחישוב כלכלי נמצא שמעבר להצלת חיי אדם, ההשקעה גם תביא לרווח כלכלי משמעותי עקב חסכון במימון טיפולי סרטן, וכוח העבודה של נשים שניצלו מהמחלה5.
  1. האם מיגור סרטן צוואר הרחם אפשרי בישראל?
    כן! אנחנו אחת הארצות המתקדמות שבן קיימת סריקת צוואר הרחם וטיפול איכותי בממצאים בצוואר הרחם. כמו כן, החיסון ניתן בישראל במסגרת שגרת החיסונים בכיתה ח', לבנות ובנים. אך לצערנו, שיעור ההתחסנות בקרב תלמידי ותלמידות בית הספר לא עלה על 60% עוד לפני תחילת מגיפת הקורונה6.
  2. מה הסיבות להימנעות מהתחסנות מלאה כנגד HPV, בישראל?
    • החשש שהחיסון אינו בטוח – זהו חשש מופרך, שכן בטיחות החיסון נבדקה אחרי שהוזרקו 270 מיליון מנות, וארגון הבריאות העולמי מצא שהוא חיסון בטוח ביותר. תופעות לוואי קשות שבעבר יוחסו לחיסון, הוכחו בהמשך שאינן קשורות למתן החיסון עצמו7.
    • במספר חוגים, הועלה החשש שהמלצה להתחסן תפורש כאישור למתירנות. גם חשש זה לא מבוסס שכן מחקרים רבי משתתפים מצאו שאין קשר בין הדברים8. לאחרונה יצא מכון פוע"ה – המכון לפוריות ורפואה על פי ההלכה – בהכרזה כי: " ...מכון פוע"ה, בהתייעצות עם גדולי הרבנים ובכירי הרופאים... ממליץ לתת את החיסון בכיתה ד', המלצה זו עדין לא התקבלה ע"י משרד החינוך. עד שתתקבל, ניתן להתחסן בכיתה ח' בלא להבליט את המשמעות החינוכית של החיסון..."9.
    • הימנעות מחיסון הבנים: הטענה היא כי "לבנים אין צוואר הרחם, אז למה לחסן כנגד סרטן צוואר הרחם?" לכך יש להשיב כי החיסון ניתן לבנים כדי למנוע מחלות הנגרמות על ידי HPV ופוגעות בבנים עצמם, כמו סרטן הפין ופי הטבעת, סרטן פה ולוע – שלאחרונה שכיחותו עולה, ויבלות באברי המין – המאובחנות כל שנה ב-20,000 נשים וגברים10.
      בנוסף, חיסון הבנים מסייע ב"חיסון העדר" ויוריד את היארעות סרטן צוואר הרחם בנשים11.
    • היסוס מצד הרופאים: כשההורים מקבלים זימון לחיסון לילדיהם, הם מתייעצים פעמים רבות עם רופאי המשפחה או רופאי ילדים. מחקרים מצאו שדעת רופאים לגבי חשיבות החיסון היא המשפיעה העיקרית על ההיענות12. כשהרופא מביע עמדה ברורה ובטוחה ביעילות ובטיחות החיסון, שיעור ההתחסנות יעלה באופן משמעותי.
    • חוסר ידע וחוסר מודעות: קיימים אתרים אמינים של חברות מדעיות ושם ניתן לקבל מידע על הנגיף, המחלות הנגרמות על ידו והחיסון. סיפוריהן של נשים שחלו בסרטן צוואר הרחם יכולים לסייע בהגברת המודעות13.

לסיכום, הכרזת ארגון הבריאות העולמית3 האירה את הזרקורים על דרך למיגור מחלת סרטן צוואר הרחם, ומחלות נוספות הנגרמות על ידי הנגיף. בידינו להגביר את המודעות למניעת המחלה, וביחוד לחשיבות החיסון כנגד HPV, ולהקפיד על הפניית המטופלות לסריקת צוואר הרחם וטיפול מתאים.

Indications:

GARDASIL is indicated in females aged 9 through 45 years* for the prevention of cervical, vulvar, and vaginal cancer, precancerous or dysplastic lesions, genital warts, and infection caused by Human Papillomavirus (HPV) Types 6, 11, 16, and 18 (which are included in the vaccine).

GARDASIL is indicated in males 9 through 26 years* of age for the prevention of anal cancer, precancerous or dysplastic lesions, external genital lesions and infection caused by HPV types 6, 11, 16, and 18 (which are included in the vaccine).

Contraindications

Hypersensitivity to the active substances or to any of the excipients.

Individuals who develop symptoms indicative of hypersensitivity after receiving a dose of Gardasil should not receive further doses of Gardasil.

Administration of Gardasil should be postponed in individuals suffering from an acute severe febrile illness. However, the presence of a minor infection, such as a mild upper respiratory tract infection or low-grade fever, is not a contraindication for immunization

Precautions/ Warnings

Special warnings and precautions for use

The decision to vaccinate an individual should take into account the risk for previous HPV exposure and potential benefit from vaccination.

As with all injectable vaccines, appropriate medical treatment should always be readily available in case of rare anaphylactic reactions following the administration of the vaccine

Syncope (fainting), sometimes associated with falling, can occur following, or even before, any vaccination, especially in adolescents as a psychogenic response to the needle injection. This can be accompanied by several neurological signs such as transient visual disturbance, paraesthesia, and tonic-clonic limb movements during recovery. Therefore, vaccinees should be observed for approximately 15 minutes after vaccine administration.

As with any vaccine, vaccination with Gardasil may not result in protection in all vaccine recipients.

Gardasil will only protect against diseases that are caused by HPV types 6, 11, 16 and 18 and to a limited extent against diseases caused by certain related HPV types.

Therefore, appropriate precautions against sexually transmitted diseases should continue to be used.

Gardasil is for prophylactic use only and has no effect on active HPV infections or established clinical disease.

Gardasil does not prevent lesions due to a vaccine HPV type in individuals infected with that HPV type at the time of vaccination.

The use of Gardasil in adult women should take into consideration the variability of HPV type prevalence in different geographical areas.

Vaccination is not a substitute for routine cervical screening. Since no vaccine is 100% effective and Gardasil will not provide protection against every HPV type, or against existing HPV infections, routine cervical screening remains critically important and should follow local recommendations.

Safety and immunogenicity of the vaccine have been assessed in individuals aged from 7 to 12 years who are known to be infected with human immunodeficiency virus (HIV). Individuals with impaired immune responsiveness, due to either the use of potent immunosuppressive therapy, a genetic defect, or other causes, may not respond to the vaccine.

This vaccine should be given with caution to individuals with thrombocytopaenia or any coagulation disorder because bleeding may occur following an intramuscular administration in these individuals.

Long-term follow-up studies are currently ongoing to determine the duration of protection.

There are no safety immunogenicity or efficacy data to support interchangeability of Gardasil with other HPV vaccines.

Clinical Significant Drug Interactions

In all clinical trials, individuals who had received immunoglobulin or blood-derived products during the 6 months prior to the first vaccine dose were excluded.

Use with other vaccines

Administration of Gardasil at the same time (but, for injected vaccines, at a different injection site) as hepatitis B (recombinant) vaccine did not interfere with the immune response to the HPV types. The seroprotection rates (proportion of individuals reaching seroprotective level anti-HBs >10 mIU/ml) were unaffected (96.5% for concomitant vaccination and 97.5% for hepatitis B vaccine only). Anti-HBs geometric mean antibody titres were lower on co-administration, but the clinical significance of this observation is not known.

Gardasil may be administered concomitantly with a combined booster vaccine containing diphtheria (d) and tetanus (T) with either pertussis [acellular, component] (ap) and/or poliomyelitis [inactivated] (IPV) (dTap, dT-IPV, dTap-IPV vaccines) with no significant interference with antibody response to any of the components of either vaccine. However, a trend of lower anti-HPV GMTs was observed in the concomitant group. The clinical significance of this observation is not known. This is based on the results from a clinical trial in which a combined dTap-IPV vaccine was administered concomitantly with the first dose of Gardasil. The concomitant administration of Gardasil with vaccines other than the ones above has not been studied. Use with hormonal contraceptives: In clinical studies, 57.5% of women aged 16 to 26 years and 31.2% of women aged 24 to 45 years who received Gardasil used hormonal contraceptives during the vaccination period. Use of hormonal contraceptives did not appear to affect the immune response to Gardasil.

Clinical Significant Use in Specific Populations

Pregnancy: The data on Gardasil administered during pregnancy did not indicate any safety signal. However, these data are insufficient to recommend use of Gardasil during pregnancy. Vaccination should be postponed until completion of pregnancy. Breastfeeding: Gardasil can be used during breast-feeding. Fertility: Animal studies do not indicate direct or indirect harmful effects with respect to reproductive toxicity No effects on male fertility were observed in rats.

Posology

Individuals 9 to and including 13 years of age

Gardasil can be administered according to a 2-dose schedule (0.5 ml at 0, 6 months)

If the second vaccine dose is administered earlier than 6 months after the first dose, a third dose should always be administered.

Alternatively, Gardasil can be administered according to a 3-dose (0.5 ml at 0, 2, 6 months) schedule. The second dose should be administered at least one month after the first dose and the third dose should be administered at least 3 months after the second dose. All three doses should be given within a 1-year period.

Individuals 14 years of age and older

Gardasil should be administered according to a 3-dose (0.5 ml at 0, 2, 6 months) schedule. The second dose should be administered at least one month after the first dose and the third dose should be administered at least 3 months after the second dose. All three doses should be given within a 1-year period.
